Rui held out the slice and, hesitantly, Tsukasa took a bite.
After besting the general in battle, Rui pays him a visit in his cell a few days later.

He never wanted to get his beloved sister sick again and force her to be hospitalized so he always took the extra safety steps to ensure that she would never have to deal with that again. But through doing this he realizes that he too, is getting sick but it’s more serious then he thought it would be. Saki doesn’t realize this and lashes out at her brother for being overprotective— she regrets her words but believes there’s hope.
Or.. Tsukasa ends up getting Saki’s sickness that she had in her childhood and still worries for his sister.
