In the aftermath of the war, Izuku has recovered physically, but there are still ghosts haunting him, whispers saying he didn't do enough. Every second, he's haunted by a feeling he can't get rid of, and nobody notices how much pain he's in.
But Katsuki notices. His friends, old and new, notice. And a whole lot more people than he'd realized. And they're not going to give up on him, because as long as Izuku’s still fighting for his life, the battle isn't over.

The blood dripping down from his limbs had made a small pod on the white tiles by the time he was done. He threw his body in the bathtub, the water burned his wounded skin but he couldn’t help himself.
Izuku couldn’t stop.
So he let them burn. The water got hotter minute by minute. He just stared blankly. It felt right. It felt so right for them to hurt, it felt so right for him to be in pain.

After the war with Shigaraki and All for One, Izuku is traumatized but Aizawa and Katuski are there for him when he spirals out of control.
