Summary
Raphael imagined himself as the supreme ruler of all Nine Hells and wanted to make it a reality. He manipulated you into a devil deal in which he would give you the Orphic Hammer in exchange for the Crown of Karsus. After you defeated the Elder Brain, you delivered the crown to him and went your separate ways. He began his attack on Avernus while you settled down in Baldur’s Gate. You thought you saw the last of him until he appeared to you in his devil form while you were having a nice glass of wine near the hearth. It was but a projection of himself since his real body was trapped in Cania, the eighth layer of the hells, the domain of the Archdevil Mephistopheles.
(Raphael is imprisoned by his father, Mephistopheles, and needs you to free him in exchange for one request that can be anything you desire.)
