Donnie’s reassurance and soft embrace calming Mikey’s tears, or Leo holding his smaller frame, tracing shapes into his shell, or Raph’s gruff but soft words and protective hug, ready to fight anything that hurts his baby brother. Mikey always felt safest in the arms of his brothers.
Now though, when a nightmare seizes his chest and constricts his breathing and his head reels, he has to bear it, crying into his pillow and stuffed animals, rather than risk annoying his elder brothers.”

Donnie always imagined that after Leo came back, everything would be okay again. Instead, his friendship with Raph stays rocky, his schedule gets even harder to manage, it still feels like he's the only one looking after Mikey and now there's a stranger living with them. It's getting hard to imagine how he could get any more stressed.
Then he wakes up with an awful headache, restrained, in an unfamiliar laboratory. Being restrained or experimented on would be bad enough on its own. Add to the mix that lying on the lab table for days and weeks is the first chance he's had to process everything else that's happened in his family--well, it's not a happy mix.

Summary
He wasn't breaking things with ill intent, and he wasn't doing it on purpose at all. He just needed something to distract himself with or else he'd go insane.
So he turned on the TV as loud as it would get and let himself move freely and let out all the jittery anxiety thrumming through his body. And sometimes, when he moved freely like that, he broke things. He wasn't actively trying to, but the sound of glass smashing and wood creaking with his weight was very satisfying and he didn't put any effort at all in trying to stop himself from breaking things.
But Leonardo just didn't understand! And Michelangelo didn't know how to put it into words for him, he had tried many times but in the end Leo just ended up confused while Mikey got increasingly more frustrated with how he just couldn't explain.
Or: Michelangelo breaks things when he's stressed. Leonardo helps him find better ways to deal with his emotions.

Unfortunately, insecurities rest in the mind a lot easier when you're sick. Good thing Mikey has one mother hen of a brother to set him straight.
